As Student Life is approaching, it has come to our attention that we did not include the Student Life Participation Forms for you to complete! These forms are a requirement for us to bring you as a leader this summer.
We are so sorry for the inconvenience!
Attached is the form and a notary is required for these forms.
Eden Hines is a notary but has limited availability due to travel/vacation. Please see if there is a time that works for your schedule:
Eden will NOT be available on Sunday, June 27
Eden WILL be available on Monday afternoon and Tuesday (June 28th and 29th)
Eden will NOT be available from June 30 - July 6
Eden WILL be available on Wednesday, July 7
The last day we can accept forms for camp is Wednesday, July 7. Of course, you can always find another person to notarize these forms as well.
